If you have received a letter from OHSU, Portland Adventist, and/or your treating physician regarding UHC contract issues, and you are currently in treatment for a serious health condition, you have the option to request "Continuity of Care" with your treating physician.
- UHC Continuity of Care Form: This form allows UHC members to request extended coverage from their current, out-of-network health care professional at network rates for a limited time due to a specific medical condition, until a safe transfer to a network health care provider can be arranged.
- Prior Authorization: Providers and members can also submit a prior authorization for services by following UHC’s prior authorization processes.
Please note: Both options above are subject to UHC’s approval.
UHC has confirmed that OHSU negotiations are ongoing, with regular discussions happening. UHC’s goal is to keep their network as consistent as possible and for OHSU to remain in-network.
To request the form, please call the TriMet Benefits Department at 503-962-7625 or the Union office at 503-232-9144.
If you have additional questions, please call Leith, our Benefit Specialist, at 360-931-0765.
